brutally honest. Yup. That is what I had to do with my wife. It hurts her at first, but 100% necessary.

I am misunderstand one point I think, he mentioned that he recommended abstinence? If they are not married, wouldn't pre-marital s be a sin?

Yes premarital sex is a sin. In the US and Europe it is a huge problem and it is viewed as normal. So having to tell an unmarried couple to abstain from it to deal with the porn issue is necessary.
